The SHO Report

Below is the website to fill in the SHO report for NHS Appraisal of Doctors in Training.

This is part of a national scheme run by the Joint Committee on GP post Graduate Training.

The scheme started in February 2005, and the GP Education team will be happy to guide you through the process with the help of your named educational supervisor, in your present post. You will need to gather feedback from patients you see and the supervisor will gain info from a 360 degree appraisal of views from your colleagues.

Please note that completion of the SHO Report is a separate process from the Appraisal that each UK doctor, including those undergoing training in hospitals, will need to complete once a year. See separate section on appraisal for SHOs for details
